In 2016 the Centers for Medicare and Medicaid services (CMS) outlined a new Comprehensive care for joint replacement (CJR) model and we are proud to participate! This model creates a stronger overarching plan of care between the physician, surgeon, home health agency, and the patient.
This new model is key to a successful recovery. Our CJR services help patients who have received hip and knee replacements. All joint replacement patients require recovery and rehabilitation periods. Our team helps to facilitate the plan of care to give our patients a better experience, and prevent infections, readmission to the hospital, and a prolonged recovery period.
